Avalon Health Center offers an innovative approach to the following:

• Fine Residential Living – Avalon is the newest Skilled Nursing & Rehabilitation Center in the area AND is the only one that is part of a full-service senior living community campus. What that means to you is that Avalon residents enjoy the same residential environment, fine dining, and attention to detailed, quality living as the independent living residents of StoneRidge.
• Convenient Location – Avalon is centrally located between Backus, L&M and Westerly Hospitals, which provides easy access to physicians’ offices for follow-up appointments or to take advantage of Avalon’s Out-Patient Therapy programs. Situated just off Jerry Browne Road, Avalon is convenient for family and friends to visit residents.
• Out-Patient Therapy – When those participating in Physical, Occupational or Speech therapy are well enough to return home but not quite finished with the full course of therapy, Out-Patient Therapy at Avalon provides an invaluable continuity of care with the same therapists and same individualized program.
• Respite Care – When caregivers need to be away for reasons such as work, vacation or family affairs, Avalon can offer their loved ones a care-filled, supportive environment for as long as needed. While they are away, caregivers can have peace-of-mind knowing that their loved one is in good hands and respite residents can enjoy all the services and activities of Avalon in an atmosphere of acceptance and friendship.
• Hospice Care – Avalon provides a tender and supportive environment for terminally ill residents and their families. As a convenience for families, resident accommodations include a kitchenette and generous seating to provide as much comfort as possible. A trained and caring staff attends to resident needs and provides pain management.
• Medical Director – Avalon’s Medical Director makes regular rounds and is contacted for any change in resident condition. The family representative is also kept updated.
• On-Staff Social Services Director – Avalon has an on-staff Social Services Director to serve as a reliable resource for the many programs, services and options available in the community for a continued, active lifestyle after discharge.
• Medicare Certified – Let your hospital or medical provider know you want your post-surgery and recovery to be at Avalon, where our goal is for residents to “Live Well. Be Well.”
