What Does It Mean to Have a Full Continuum of Care at a Life Plan Community?

While the future can be hard to predict, with the right plan it can also be exciting. That’s why every day thousands of older adults start looking for a senior living option that lets them continue to enjoy the life they love today with a smart plan for tomorrow. They recognize the benefits of living an active lifestyle backed with a full continuum of care.

What Does Continuing Care Mean?

A senior living community that offers a full continuum of care — like StoneRidge — is called a Life Plan Community. Also known as a Continuing Care Retirement Community or CCRC, Life Plan Communities offer an all-inclusive, maintenance-free, wellness-focused lifestyle with plenty of social opportunities and activities.

At StoneRidge, you can move into an independent living apartment home and start enjoying a full range of helpful services and thoughtful amenities right outside your door, including multiple dining venues, a heated indoor saltwater swimming pool, auditorium with surround sound, creative arts studio, resident art gallery, woodworking shop, fitness center, social calendar, and so much more. We also offer a continuum of care — like assisted living, memory care, skilled nursing and rehabilitation — right on-site.

Benefits of Continuing Care

Some of the benefits of choosing a senior living community with a continuum of care include:

  • Greater stability: If you move to a community without continuing care, should you need a higher level of support, you may have to move to a different community during a health emergency.
  • Personalized care: Once you become a part of a community, you get to know the team members and trust the services they provide, and they get to know you. This level of comfort and familiarity becomes even more important if you need additional care and support.
  • Peace of mind: Continuing care offers reassurance to you and your family. Plus, knowing you have access to on-site care, when needed, allows you to focus on today with less worry about the what-ifs of tomorrow.
  • Live with your spouse: If you or your spouse needs care, some communities offer assistance in your independent living residence (see below) or on the same campus. So you’re never far apart.
  • Stay in a community: On-site care allows you to stay connected with your friends.
  • Keep enjoying life: You’ll continue to have access to the community’s services and amenities.

What Are the Different Levels of Living?

Assisted Living

This level of living is designed for older adults who need a little help with the activities of daily living, such as bathing, grooming, medication management and transportation. For older adults who are finding it more and more difficult to take care of a home (and themselves), assisted living can provide security and peace of mind. StoneRidge offers two levels of assisted living to help you maximize your independence:

  • SAIL: StoneRidge Assistance-in-Living with light support services offered right in your independent living residence.
  • TopSAIL: StoneRidge Assistance-in-Living with a higher level of support in one of our dedicated assisted living apartments.

Memory Care

Memory care communities provide a safe, secure environment that’s designed for the needs of those with Alzheimer’s disease or other forms of dementia. Team members are trained in best practices and focus on celebrating the person, not their condition.

Skilled Nursing

With 24/7 care available, skilled nursing is for those who require long-term care for chronic conditions like advanced diabetes or degenerative illnesses such as multiple sclerosis or Parkinson’s disease. Short-term rehabilitation is also available following an illness, accident or surgery.
